Thanks for pointing out the duplicate use of that id string.  I'm a bit
disturbed that the warning goes to STDOUT.  I don't get this warning at
all on my system, even in the ScrollKeeper log file where warnings are
typically sent.

Do you get the error if you do:
(a) /usr/bin/scrollkeeper-extract 
/usr/share/scrollkeeper/doc/writing_scrollkeeper_omf_files/C/writing_scrollkeeper_omf_files.xml 
/usr/share/scrollkeeper/stylesheets/index.xsl /tmp/foo

or

(b) /usr/bin/scrollkeeper-extract 
/usr/share/scrollkeeper/doc/writing_scrollkeeper_omf_files/C/writing_scrollkeeper_omf_files.xml 
/usr/share/scrollkeeper/stylesheets/toc.xsl /tmp/foo2

Which versions of libxml2 and libxslt do you have?
