Most users are used to the "File" menu being the leftmost toplevel
menubar entry. Putting a "per app UI customization menu" there breaks
the "feel" of the interface.
Toplevel Menuentrys waste screen real-estate, and should actually be usefull
most of the time, whereas your suggestion would be usefull maybee to only 1/3
the users for a very short time (once its configured correctly you don't need
it anymore, and most users won't bother to customize the UI anyway).
I think that stuff belongs into the apps "Settings" menu, but that propably
hard to implement uniformly. If it's something that customizes
a toolbar/menubar it should go into a rightclick contextmenu.
all just IMHO of course...
