Toolbar toggle and radio items

Michael told me we haven't had enough API breakage lately, so I just
checked in a patch to add pixmap_type and _data parameters to the UIH
*_toolbar_new_toggleitem and *_toolbar_new_radioitem functions.  I'm
going to take a look at radioitems and see if I can get them implemented
in the current framework.
