Yes, there are caveats. You're right it won't recompile older packages
against the newer stuff. It'll only compile the new stuff.

Yup.

Yes, this is the best option but not everyone wants to recompile
everything from scratch every time there is a minor update. For most
things you should be able to just run a 'make paranoid-install' after an
update and things should hopefully work fine. But you're right if core
things change then a 'make clean' would be the best solution.
