Sorry if I come two years late, but in case this can help others:

I just had this exact same problem on OpenBSD 4.6-current with packages:
- evolution-2.28.2p0
- evolution-exchange-2.28.2

I succedded in getting rid of the "Permission denied" errors and adding the calendars. The problem came from the fact that I'm working in France, so our Exchange Server is in French, therefore the access to the calendar should done through the URL:
http://<server>.fr/exchange/fname.lname/*Calendrier*/
Whereas my evolution/OpenBSD is configured in uk and therefore evolution's dialog box that appears when clicking on "File/Subscribe to other user's folder" is showing "Folder Name: *Calendar*/Inbox/Tasks/Contacts".

Writing "Calendrier" instead of "Calendar" in the list does the trick.

However, even if some calendars are added painlessly, I still have problems for accessing some others. Some of the problems are clearly related with the fact that we have 2 exchange servers and that I'm not on the same as those that give errors (but a web access makes a transparent redirection. In this case the calendar is added, but I can't see the content). Other errors seems related to the fact that the colleagues provided different sets of permissons (but reading is there since web access works. In this cas, the "Permission denied" is still there).
