Hello,
please check if you've checked in Edit->Preferences->Calendar and Tasks,
tab "Alarms" your Exchange account. Just in case... Otherwise I don't
know what's going wrong for you, it works for me, so maybe this option
will help you.

Today actually I got an alarm in the notification area for the first
time ever. I now know that I have never seen that before. This is not
the normal behavior though; If I add a new appointment and use alarm on
it it won't show.
If I add entries to my personal calendar instead of the calendar via
Exchange I can see them in the clock integration. How can I make this
show my appointments from the Exchange calendar instead of my personal?
