[Epiphany] "exit fullscreen" button

This button should be dynamically added to the toolbar, not placed over the 
rendered content as an extra button.  go to http://robin.sourceforge.net/ 
for a graphic example of why having the button over the content is a really 
bad idea.  I guess it is easier to find there but the added easy of location 
does not seem worth it.
