No, this is from epiphany from the 1.1.2 tarball. I just changed to the
Default gtk-2.0 theme and the Gnome icon theme, with no change.
FWIW the following icons in the Toolbar Editor dialog are also not
showing up (the white box with the red x)

New Tab
Can someone tell me what themes they are using, since I have tried many
with no luck. I just need to know if this is an epiphany bug, or if a
theme bug so I can file something.
