gnome-volume-control accessibility patch

Hi All,

This patch adds accessibility support to gnome-volume-control. It adds accessible names to all the sliders without changing the UI, and changes the tab order such that the checkbuttons for mute, rec, and lock directly follow the corresponding sliders. IMHO, this is more useful than having all the mute, lock, and rec buttons grouped together in the tab order. BTW: this patch makes the mixer completely useable with speech with my simple talking test app.
