Attachments are included in the mail index.  When you run best, do you
see the string "(email attachment)" afterward?  If so, everything is
working normally. :)

The NullReferenceException needs looking into.  Do you think you can
send me that mbox file, so I can try to see what's causing it and how
best to fix it?

The file exceptions... probably bugs but not related to the mail stuff.
