vim: fix ctrl-[ to escape from insert mode
ctrl-[ should always be a synonym for Escape but it's only currently a
synonym for Escape in one of the two places Escape is specified.
This commit adds a duplicate entry for the other place, too, and fixes
escaping from insert mode using ctrl-[.
