Log:
Rewrite of video output routines. More code sharing between GDK,
XV and DX. Additional thread for displaying the video frames,
triggered by the local and/or remote PVideoOutputDevice.
Syncronize to higher framerate. Enable preview and receive-only
video on win32.

Align naming of functions. Minor fixes for embedded window on win32.
