* src/lgo.py: added check on <document> creation for possible
ElementTree serialization to aid in debugging ElementTree errors; also
added check on toc_id to ignore modules with toc_id as None (happens
with gnome-doc-utils modules missing their omf files, such as newly
released gnome-media-2.20.1)
