Allow monitoring of playlist changes: The "changed" signal is quite

fuzzy, and just exporting the tree-model is pointless due the useless
nature of its "row-deleted" signal.

* src/Makefile.am: Add totemplaylist-marshal.list.
* src/totemplaylist-marshal.list: Add VOID:STRING,STRING.
* src/totem-playlist.c, src/totem-playlist.h: Add "item-added" and
"item-removed" signal. Implement totem_playlist_foreach function.
