192 blocks is extremely small for an ISO 9660 session.
How many files did you add and how many bytes of content do they have ?
How many files are in the first session ?

It is normal that the add-on session is much smaller than the first one.
Nevertheless it needs to contain the new merged directory tree with the
names of all the old and new files. Further it needs to contain the data
content of the newly added files.

So, unless the total number of files is small and only a few small files
were added, the size of 192 does not look good.
It is supposed to come from mkisofs, genisoimage, or libisofs. All three
are known to be able to produce good add-on sessions if they get operated
the right way.

You would have to tell the paths of the directory trees or files which
you want to put on DVD+R and the paths which they shall have there.
