File->New->Local Maildir mailbox

or

File->New->Local MH mailbox

Then right click this Mailbox and set "Mark as xx"

(note: These possibilities should be accessible from the Menu as well!)

To change the outbox you have to edit ~/.gnome/balsa by hand (you will see 
the necessary changes!)
