I'm using gentoo too...

you also need pspell but it should already be installed...
you don't need gnome-spell (at least i haven't installed it)

Spell checking is working for me...

I'll suggest to

#emerge unmerge libesmtp
#emerge unmerge aspell
#emerge unmerge balsa

#emerge aspell
#emerge balsa
