Balsa+libESMTP+Bcc (was Re: BCC not working?)

Hi Folks

Following on from this discussion, I've made a patch to send.c from
the Balsa+libESMTP cvs.

Basically, it arranges to send the message twice when there are
Bcc recipients listed.

Recipients in the Bcc list get the message with a Bcc header
other recipients see the message without a Bcc header.

This is behaviour described in RFC 2822.

I've only just quickly hacked this patch, so there may be a few
issues with it.  I'm assuming the use of libESMTP-0.7.0
