--with-esmtp

I've added a --with-esmtp to the libESMTP branch of cvs. --with-esmtp=no
disables all remote MTA support in balsa. I've given it a spin, could the
people using /usr/bin/sendmail field test it please ? Also, people using the
libESMTP could check if i've broken anything (Brian ?)
