There are a lot of software which support nested maildirs -
courier-imap, maildrop...
I have
~/Maildir/cur
~/Maildir/new
~/Maildir/tmp
~/Maildir/.Sent

ls -l ~/Maildir/.Sent:

drwx------    2 nerijus  nerijus      5038 Lap 25 19:40 cur
drwx------    2 nerijus  nerijus        35 Lap 25 19:39 new
drwx------    2 nerijus  nerijus        35 Lap 25 19:39 tmp
-rw-r--r--    1 nerijus  nerijus      3501 Lap 25 19:39 courierimapuiddb
-rwx------    1 nerijus  nerijus         0 Geg 30  2000 maildirfolder

Any dir whose name begins with dot, has 3 subdirs (cur, new, tmp)
and has file maildirfolder is a maildir also.
So it would be nice if balsa could automatically add all submaildirs.
