[orca-list] orca does not announc window titles if they are the same

Hello,

I found an interesting but.

1. Open two applications with identical window titles, e.g. Leafpad.

2. Open third application with a different window title.

3. Alt-tab switch among those windows. Notice that if there are two 
consecutive window in the list with identical window titles, Orca reads 
only one of them.

I think this should not happen.
