seems to have solved the software engineering problem of bringing down the

well known operating system and application  vendors) or by smaller groups
with fewer resources.

The solution is probably a combination of technical and social changes,
including educational measures and more effective regulations.

That last point is very crucial.
We have filed a public litigation in Indian court for this reason.
As far as technical stuff is concerned, I guess the awareness is lacking.  I
am not talking about accessibility awareness as such, but about the fact
that there are professional blind programmers.
So the priority of accessibility bug fixing changes automatically.
