I've just filed bug #347347 ( 
http://bugzilla.gnome.org/show_bug.cgi?id=347347 )
against Evolution on this. In short, it's not generating 
"object:text-caret-moved"
events when you have the shift key down and are arrowing around. Orca needs
to see them in order to provide feedback on whether the text is selected 
on not.
