Does Gtkperl exist for MS Windows / Mac OS X?

depends on whether you mean gtk-perl (for gtk+ 1.x) or gtk2-perl (for gtk+ 2.x).

i don't know that gtk-perl was ever successfully run as a native win32 toolkit
(i.e. using the win32 api port of Gtk+). i think it is possible to compile
gtk-perl under cygwin and have it run on an X server, but this is not very
fun.

it is possible to build and run gtk2-perl on win32 with the native win32
version of Gtk+ 2.x -- check the list archives for last month for a message
from me about that. gavin brown is working on a current build. i hope to
have binaries for the 1.0 release.

similarly, i imagine you could get both versions running on OSX as X11
applications, using fink. i have never tried this nor heard reports about it.

running as a native quartz application would require a native quartz port of
gtk+, of which i have never heard.
