Doesn't matter what the setting is, just that it has to be changed.

other calendars that are there I got a bunch of notifications pop up for
the exchange calendar. I then created a test appt in the exchange
calendar and it popped up.
I've got some other appts coming up this afternoon, that will be the
test as they were created by someone else that I just accepted vs
creating locally.

Oh, I forgot to mention third option, but this one is quite obvious:
Make sure you have the exchange calendar selected in
Edit->Preferences->Calendar and Tasks->Alarms
It is selected by default, so it might not be it.
I'm mentioning it here just in case.
