I would be very interested in a perl plugin for dia.
I have loads to dia related stuff I am working on and its all perl.

Now that I think about it, I'd be interested in a Perl plugin as well.
My Dia-->SQL converter (http://tedia2sql.tigris.org) is written in Perl.
I don't imagine rewriting it to deal with Dia internal structures would
be too hard. I think someone said Dia's internal structures mimic the
XML savefile.

More like the XML savefile mimic Dia's internal structures:) It's
essentially a dump.

Would a Perl plugin allow me to add a "Save as SQL DDL" option on some
menu which then calls my Perl module?

Yes, it's called an export filter, and the Python plugin already allows
those.
