[xslt] Omitting namespace declarations from root output element

I'm getting the namespace declaration attributes, which appear in the
XSLT stylesheet element, appearing as attributes of the output document
root element i.e. <xhtml
xmlns:xsl="http://www.w3.org/1999/XSL/Transform";>

How do I stop this from happening?
