What about adding a toggle next to the app-buttons in the left pane
stating this as "Default application", then adding some rules somewhere
in the preferences for how to choose the default application. If an
user double clicks on a file icon the default app is used.

Default apps could be choosed from statistics (if the user uses Opera
98% of the time then use this as default), from what the user used last
time (perhaps even for this file), use whatever is set explicitt for
this file, etc.

I would also like to have the apps buttons in the left pane when a
file is selected, not when it is already in some viewer.
