[DEPRECATED] Suggestion for addition

I think it would be useful if the guidelines said something about when 
icons should appear in the menus, and when they shouldn't.  Some apps 
have icons for every single menu item, some have no icons whatever, some 
have icons only for items which are repeated on toolbars, and some have 
no patterns that I can discern.
