What skins are possible?

Forgive my ignorance, but I am wondering how much a skin can change
the look of an application.

Is something like the gqmpeg skins possible? See for example
http://gqview.netpedia.net/mpeg-skin.html

If not, would it be possible to extend the skin capabilities to do
something like that? Or is that too application specifc?
