gtk_item_factory_create_item - how?

I'm at a loss as to how to properly use this function.  I've looked at the
reference page
(http://www.gtk.org/rdp/gtk/gtkitemfactory.html#GTK-ITEM-FACTORY-CREATE-ITEM),
but it doesn't say much.  I've looked for sample code using it, but no luck
(lots using gtk_item_factory_create_items, of course).

Specifically, the problem is how do I make the second parameter, the
GtkItemFactoryEntry?  I tried using the format from the attr_list of the menu
tutorial, but no go.  I know I'm missng something pretty simple here, but
what?
