Using gtk-doc.make (like GTK does, for instance) is probably the best 
option going forward.

By the way, if the index.sgml file isn't getting generated on your 
system, it indicates a problem with your docbook toolchain (either SGML 
or XML, depending on which gtk-doc code path you are using).
