There is some information about how GTK+-2.0 themes work at:

Though it is:

 a) Intended for people writing theme engines
 b) Slighty out of date

GTK+-1.2 themes that are only RC files should still work with
GTK+-2.0 -- you just need to edit your ~/.gtkrc-2.0 and add
a line like:

 include "/usr/share/themes/MyTheme/gtk/gtkrc"

There aren't any fully working theme engines for GTK+-2.0 out
there yet, AFAIK.
