It works for me with at least "Left Alt" key. It's probably very
messy internals hitting in: my other keys already have different
re-definitions.
So yes, this depends on a lot of factors, and it's up to distributors
to ensure that what they ship as their xkbdata corresponds with
base.xml/xorg.xml.

Yes, I think so.

Probably the same problem: I am not exactly sure which of these
options can work with each other, but they should all work in some
configuration or the other.

Not me, but the second "Ctrl+Alt" server keys seems to be something
about killing an X server, or switching between virtual terminals ;)
