Probably the best source of information about GTK themeing will be 
found here: <http://ajgenius.us/Gnome/Themes/Tutorials/?id=GTKThemes>, 
with information specific to pixmap themeing here: 
<http://ajgenius.us/Gnome/Themes/Tutorials/index.php?id=PixmapEngine>. 
However, there are already quite a lot of GTK2 themes mimicking Mac OS 
X, so it may be easier for you to use one of those!
