This is handled automatically by libxml2, as long as the parser is given
the parameter telling it to do so.  Yelp does pass this parameter (see
line 239 of yelp-db-pager.c if you'd like), so it should work.  However,
I don't remember if I ever tested it.

I've said a number of times in the past that I would prefer that we use
XInclude rather than entities for splitting documents across multiple
files, so I certainly consider it acceptable for contribution.  If we
actually have any problems, they're bugs, and they can be fixed.
