file from a host, the page offset information is read early and is
used to download a specific page requested by the user without
downloading other pages in the document.

If the only "new" thing here is a table of contents at the beginning,
I would expect there is enough prior art for this. Meanwhile, at
least it does not endanger ordinary use of PDF. But it shows that it
would be good to find a way to wrest effective control of the format
away form Adobe.
