[PATCH] gtk-xmhtml form support

I don't know who the maintainer for gtk-xmhtml is, or if there is one, but
the forms support is seriously broken.

I have fixed the forms code to provide at least a minimal level of
functionality. No promises, but it certainly doesn't work any worse than
before (I hope).

Rather than sending everyone on the list the patches I've placed them on a
temporary webpage at http://home.att.net/~ssolon/gtk-xmhtml

I hope someone will pick them up and apply them.
