The revhistory should satisfy the history of the GFDL.  At least,
we consider it to satisfy it for Gnome docs, and the intent of the
copyright holder is golden.

As for the legal info, you're not publishing the Yelp rendering.
You, as the author, are publishing a DocBook file, and you have
maintained the legal info verbatim.  You're hardly at fault if
somebody views the document in an application that doesn't render
it properly.

Menus and menu items should absolutely be inside <guimenu> and
<guimenuitem>.  And there's no need to specify a seperator, or
even use one.  That's what <menuchoice> is for.
