If you are using a gatekeeper, both of them will register to the
gatekeeper and contact each other using the 'alias'. You have to add an
H.323 account to register to a gatekeeper.

If you are not using a gatekeeper, direct IP dialing is the only way to
get in touch. You do not have to add an H.323 account if you are using
direct IP dialing.

I guess so, I don't know any public service.
Perhaps have a look on the excellent http://www.voxgratia.org, they were
showing a Demo GK.
