I don't think this can be saved as a pref yet.

I understand it differently. I think it applies to when they read the
message.

Yep. Go to the Preferences option on the Settings menu. Go to the Mail
Options tab. The section at the bottom is for what Balsa should do if it
receives a mail with request-disposition-notification stuff in it. You can
set it to 'Never', 'Ask me' or 'Always.
