Creating remote IMAP mailboxes

How do you create a remote IMAP mailbox? I mean the real, actual
mailbox, not Balsa's mapping of the mailbox. So far, TkRat and
Netscape/Mozilla are the only two Unix mail clients I've used which can
actually create a remote IMAP mailbox (I haven't tried with pine--I
don't have it installed). It seems like this should be a basic feature
of any IMAP client.

Thanks,
