It may well be that it sends the sound too early, but it seems that "too
early" for ekiga is "just right" for some other clients.

For instance, I've noticed that Ekiga cuts off the first second or
two of the Gizmo echo test. When I try it with the proprietary
Gizmo client, I can hear everything.

I've also noticed that during the first second or so in ekiga, there's a
weird distortion before the sound stabilizes. I don't know if this is
related.
